Abstract (Max 200 words)
Purpose: To compare the higher-order aberrations(HOAs) changes after Wavefront guided (WF-G) FA-LASIK versus WF-G LASEK in the treatment of high myopia and astigmatism.
Methods: A prospective comparative case series study comprised 114 eyes with spherical component greater than -6.00 diopters (D) and cylinder lower than -3.00 D were assigned to 2 groups: 47 eyes were treated with FA-LASIK and 67 eyes with LASEK. Total RMS and HOAs value changes, Vertical and horizontal coma and spherical aberration were measured for each patient using a Shack-Hartmann wave- front sensor (Zywave, Bausch & Lomb, Rochester, NY) over a 6-mm pupil preoperatively and 6 months postoperatively.
Results: Preoperatively the mean refractive spherical equivalent (MRSE) was -7.12 ±1.47D in FA-LASIK group and -6.97 ±. 96 D in LASEK group, at 6 months it was -.27 ±. 41and -.05±. 51 respectively. At 6 months, there were no any statistically significant differences in term of UCVA P=. 13, DCVA P=. 65,Total RMSvalue P=0.29. The mean Total HOA-RMSvalue at 6 mm pupil were .360 in LASIK and .361 in LASEK group that increased to .614 and .711 respectively P=0.05. Vertical coma P=. 005 and Spherical aberration p=. 000 were increased significantly in LASEK group.
Conclusion: Both customized WF-G FA-LASIK and WF-G LASEK were safe and effectively treated eyes with high myopia and astigmatism, although HOA’s values increased significantly more in LASEK group.
